By hand, VERY carefully.
I did that with the last core release, and ended up accidentilly replacing customized code.

I started to write an upgrade utility, but haven't gotten very far with it yet.

Basicly, you want to load up your moo, load up the new core, connect to both, and start comparing code.

If there're objects you /know/ you haven't changed, you could just use one of the various porting mechanisms 
out there to bring the new version over.
