Problems Getting pppd to Start

It's somewhat hard to say what's going on. In any case, mgetty definitely
is starting pppd, and this doesn't fail (otherwise mgetty would log an
error message and exit).
You could try starting a small shell script instead of /usr/sbin/pppd that
will run "strace /usr/sbin/pppd 'normal pppd options' 2>/tmp/strace.log",
and see whether that will show something unusual.
