Sounds like "DCD is low".  Please try with "ignore-carrier true" in
mgetty.config -- if that helps, check your modem setup and cabling,
because "ignore-carrier true" is a BAD workaround - it will lead to shells
not terminating when the user's modem connection breaks.

Unfortunately, you didn't include a mgetty log file, which might have
shown enough details to nail down your problem right away.  (But users
never do, I start getting used to it).
