Well: this looks ok. Except for the "vt100", which shows that you haven't
fully read the documentation :)

This looks very bad, though. Something weird is going on, and I can't
diagnose it from here. Your serial port or serial cable may be defective
-- these kinds of errors should only happen "every now and then", not
every time you start vgetty.

"little out of sequence" makes it much harder for us to read -- so, if you
send another log file, make sure it contains meaningful information in the
proper order. Remember, we all do this in our spare time, so you must take
care to help us, and make our job easier... (or pay us for support).

The inittab entry is (mostly) ok. Dunno why it isn't working, though.
Maybe a complete log file entry (from startup to the "waiting..." line, or
to a line showing a repeatable (!) problem) would help.
