Hylafax has a Windows Client which captures Postscript output and has a
phone book. It's what I use for fax.
You could of course use any Postscript driver and spool it through a Samba
printer, so long as a) the fax number is in the document being faxed in b)
a standard format of sorts allowing you to grep it out.
