At this place, your modem complains about "DTE to DCE data underflow".
This is something that I have occasionally seen in badly programmed
modems. Usually the modem should always insert enough padding to handle
even slow computers.
Anyway, I think it should help to use "speed 38400" in sendfax.config (and
no "switchbd" statement in that file!)

This is a followup problem, sendfax doesn't really expect the connection
to be dead already, you only notice when reading the log file [this is
kind of a "can't happen" thing].
