mgetty process not respawned

I am using Slackware 2.0.34. mgetty 1.1.9 is added to /etc/inittab:

d1:12345:respawn:/usr/local/sbin/mgetty -D -x2 /dev/ttyS0

After rebooting the OS, I don't see process mgetty from command:

ps -a

I tried to dial to this Linux box and this modem connected to com1 is not
pick up by any process. However, if I issue command by hand:

/usr/local/sbin/mgetty -D -x2 /dev/ttyS0

then, the phone is pick up though the login is rejected somehow.

What goes wrong to my /etc/inittab or mgetty?
