Mgetty Not Running

My Linux 2.0.34 has mgetty1.1.18 entry in /etc/inittab:

d1:12345:respawn:/usr/local/sbin/mgetty -D -x2 /dev/ttyS0

process mgetty is invisible by command "ps -a" after rebooting the OS.
Natually the phone call to the system is not pick up by the system. I have
changed the runlevel to others but still can not see mgetty's existence.

However, if I manually issue the command

/usr/local/sbin/mgetty -D -x2 /dev/ttyS0

then, the system picks up the phone calls though the caller and the
called failed to estabilish ppp connection.

What goes wrong to my /etc/inittab (or mgetty)?
