Error when sending fax file...

Hello, I am using a MutliTech MultiModem Model # MT5634ZPX-PCI.    I am having a problem when sending a fax.  I have looked through the last 3 months archive for a similar error, but have found nothing.  This may be as simple as modifying the init string, but I am not sure.  Could someone please look at the below sendfax.log file and tell me what's up?   I would appreciate any help.
