This seemed to work for me.

No the timestamp file is created to show when the messages were last
listened to.
You need to put dtmf.sh and a file .code in /var/spool/voice. .code
should just contain the numbers you want to type in to activate the
playback.
You will probable also want to change voice.conf so that it doesn't
keep the recorded message once it sees dtmf.
The problem with using dtmf_program and the default configuration for
how the systems plays the outgoing message and then starts recording
is that dtmf recognition doesn't seem to be enabled until the outgoing
message has been played. With my USR sportster, the dtmf code
recognition seems a bit intermittent. The response also seems to be a
bit slow (play message, enter dtmf tones, pause, play message, beep,
pause etc)
