That's trivial :) - "init-chat"

You want to use 1.1.18 and "cnd-program".

The cnd-program called would then write a logfile (or broadcast the
telephone number, or whatever), and then tell mgetty "NO DON'T PICK UP!".

Like this...?

Nov 6 17:34:11 greenie syslog: ISDN CALL: tty: tty5d from 'none' to '35655024'
Nov 6 17:38:26 greenie syslog: ISDN CALL: tty: tty5d from '89747XXXXX (Woba)' to '35655025'
Nov 6 22:00:27 greenie syslog: ISDN CALL: tty: tty5d from '89323560 (SpaceNet)' to '35655024'

(My cnd-program calls syslog, which is set up to broadcast the calls onto
all my virtual ttys, so that I can see who's calling my home phone when
I'm logged in from work...)
